This works by inhibiting PDE5, an enzyme that breaks down cyclic guanosine monophosphate (cGMP) in smooth muscle of the corpus cavernosum of the penis and in the pulmonary vasculature.
By inhibiting PDE5, tadalafil enhances the effects of nitric oxide (NO) released during sexual stimulation, leading to increased cGMP, smooth-muscle relaxation, vasodilation, and increased blood flow.
In the case of PAH it relaxes pulmonary vasculature, lowering resistance and improving blood flow.
